Live-in care uses one caregiver (sometimes two, alternating) who sleeps in the home and is available if needed but is not required to be awake and active all night, which suits a person who is safe overnight but should not be alone in the house at all.
Choosing between 24-hour and live-in care
The deciding question is almost always: does this person need someone awake and responding through the night, or just present? If falls, wandering, agitation or physical needs happen overnight, 24-hour shift coverage is the safer plan, because a live-in caregiver who is asleep cannot respond the way a shift caregiver can. If nights are generally quiet and the concern is daytime safety plus not being alone in an empty house, live-in care is usually the more comfortable and more affordable answer.

How scheduling actually works
24-hour care is staffed as shifts — typically two or three caregivers rotating across a day, each alert and working their full shift, with a consistent small team rather than a rotating cast. Live-in care assigns one caregiver who lives in the home for a set stretch of days, with defined rest periods, and a second caregiver covering their days off.
Both arrangements require a private bedroom for the caregiver. We will walk through the specifics of the house during the assessment, because the logistics of live-in care depend on it in a way lighter care plans do not — a spare bedroom, a fold-out couch in a shared living space, and a basement guest room all change what a workable schedule looks like.
Paying for 24-hour or live-in care
Round-the-clock care is the most expensive form of home care because it involves either multiple full shifts or a dedicated live-in caregiver, and we will not publish a number here because it depends on the arrangement — the exact structure is priced on the call after the assessment. What does apply broadly: private pay, long-term care insurance, and VA Aid & Attendance for eligible veterans and surviving spouses. For families who qualify, Serenity's PASSPORT agreement with the Council on Aging of Southwestern Ohio — personal care, homemaker services and structured family caregiving, across all five counties Serenity serves — can apply toward the authorized hours, though PASSPORT authorization for true round-the-clock coverage should be discussed directly, since the covered hours are set case by case.
Medicare does not cover non-medical 24-hour or live-in care, regardless of how many hours are involved.
