Delete Your Account & Data

Applies to the Serenity Care mobile application, operated by Serenity Care Partners LLC (published on Google Play by Blue Haven Consultants LLC).

This page explains how a user of the Serenity Care app (a workforce application for caregivers and staff of Serenity Care Partners) can request deletion of their account and associated personal data, what is deleted, and what we are legally required to retain.

The fastest way to delete your account is inside the app: open Profile → Delete My Account and confirm. You do not need to contact us first.

How to request deletion

  1. In the app (recommended): Sign in, go to Profile → Delete My Account, review the confirmation screen, and confirm. This removes your app account and access and submits your deletion request, which we complete as described below.
  2. By email: Email Care@serenitycarepartners.com (or Hello@serenitycarepartners.com) from the address on your account with the subject line "Account Deletion Request." Include your full name so we can verify your identity.
  3. By phone: Call (513) 400-5113 and ask to submit an account deletion request. We may verify your identity before processing.

We confirm receipt of email and phone requests within 5 business days and complete eligible deletion within 30 days.

How the Serenity Care app works. The app is the field interface to our clinical and operational system of record. Information you create or send through the app — for example, visit documentation, Electronic Visit Verification (EVV) data, and care-team messages — becomes part of a care, service, or employment record. Deleting your account removes your account and your access to the app; it does not erase records that law requires us to keep, which are described in the next section.

What is deleted

When you submit a deletion request, we:

About your sign-in. Your Serenity Care login is also the credential you use to reach our secure operating system (the ERP) that the app connects to. If you are a current employee or contractor, that credential is tied to your employment and is deactivated in line with your employment status (for example, upon separation). Deleting your app account removes your app account and access; to coordinate removal of any remaining employment-related access, contact us at Care@serenitycarepartners.com.

What data is retained, and why

Serenity Care Partners is a licensed Ohio home-care provider. Some records generated through the app are records we are legally required to retain under Ohio Medicaid rules (Ohio Administrative Code 173-39-02(A)(3)), the HIPAA Security and Privacy Rules, and federal/state employment and tax law. These records are retained after account deletion for the periods below, then securely destroyed (cross-cut shredding for physical records; certified data destruction for electronic records):

We keep retained records only for the compliance purpose above, restrict access to authorized personnel, and securely destroy them at the end of the applicable retention period. We do not use retained records for advertising, and we do not sell them.

Protected Health Information (PHI): Patient care information handled through the app is governed by our HIPAA Notice of Privacy Practices, which controls how PHI is retained, disclosed, and destroyed. Deleting your app account does not override HIPAA retention obligations.

Note: If you are a current employee or contractor of Serenity Care Partners, deleting your app account removes your access to the app but does not end your employment records, which are governed by the retention rules above.
